UAW-GM MARTINSBURG
FITNESS CENTER

To all Current Active and Retired Employees of GM-SPO Martinsburg:

We are pleased to announce the opening of the UAW-GM Martinsburg Fitness Center. The opening date is October 20, 2004 at 9:00am. The Center is located at the East End of Plant 50 in the old fluid room. You must enter through the East entrance (old East hourly). The Center will be open seven days a week, twenty-four hours a day, unless posted otherwise. Using the Center during your working hours is prohibited except during your breaks or lunch.

Membership is limited to current active and retired employees of GM Service and Parts Operations, Martinsburg. The Membership fee is twelve dollars per year.

To join the Fitness Center follow Step 1 and 2 below:

Step 1:
In order to join, active employees must contact the Nurse at your respected plant to have your blood pressure checked and to obtain a waiver form. Employee must sign the waiver form. Retirees must take the form to their personal physician and have their blood pressure checked. The waiver form must be signed by the retiree and stamped and initialed by a personal physician. After the signed waiver form has been obtained - call Phil Mackey, ext. 267-5666 or Harry Mundy, ext. 267-5667 to arrange for an appointment (between the hours of 6:00am until 2:00pm).

Step 2:
Bring the waiver form with you to your appointment and a personal check for your dues ($12.00), the check is to be made out to UAW-GM Fitness Center (checks only - cash will not be accepted). Dues will not be refunded due to cancellation or loss of membership. Your waiver form will be taken and it will be returned to the Nurse where it will be placed in your medical file. Retiree's forms will be filed in Fitness Center correspondence files. Within a few days your badge will be activated to allow you entrance through the East entrance and the Fitness Center.

Showers are available; however you are to provide your own accessories and towels. Lockers are available and you may contact security in Plant 50 to obtain a locker.

Because you are on GM property the Shop Rules will apply. Violation of the rules can result in discipline and the loss of Membership. Improper use of equipment or inappropriate conduct can result in loss of Membership.
